How do I get started to refer a LCP solution request?

Complete the Consent to Release Authorization Form and have the Claimant sign and return it to you as soon as the file is referred to us for the LCP.

Complete the Referral Form and attach it to the front of your prepared claim file materials.

Include the last 2 years of Medical Reports from all treating medical providers including all Independent & Disability Medical Exams. Also include medical documentation supporting Maximum Medical Improvement (MMI) where it is available.

Send a copy of the proposed Settlement Documents (if applicable).

Send a copy of any annuity documentation identifying lifetime benefits (if applicable).

Complete the Referral Form;
