How do I get started to refer a MSA solution request?

Check your file to see if it is a Medicare/Medicaid Set-Aside candidate.

Complete the Referral Form and attach it to the front of your prepared claim file materials.

Complete the Consent to Release Authorization Form and have the Claimant sign and return it to you as soon as the file is referred to us for the MSA.

Include a payment history of all Indemnity, Medical and Prescription drug payments for the last 2 years.

Print a copy of the First Report of Injury.

Include the last 2 years of Medical Reports from all treating medical providers including all Independent & Disability Medical Exams. Also include medical documentation supporting Maximum Medical Improvement (MMI) where it is available.

List any Denied Body parts (on your company letterhead).

Send a copy of the proposed Settlement Documents (if applicable) with Medicare language to obtain MSA approval.

Send a copy of any annuity documentation identifying lifetime benefits (if applicable).

Complete the Referral Form;
